Waiver of Liability:
I understand that the art of special makeup effects involves the
use of many types of materials and/or tools, some of which can
be harmful if not used properly or can cause allergic reactions
to some. I hereby waive any right to hold 1313FX responsible for
any such bodily harm, damage or reaction that may occur while
taking part in this workshop.
Use
of Likeness;
I hereby grant to 1313FX the right to use images of myself and
my work in connection with the advertisement, exploitation and/or
publicizing of 1313FX workshops. I agree that I will not assert
or maintain any claim, action, suit or demand of any kind, including
but not limited to those grounded upon invasion of privacy, rights
of publicity or other civil rights, or for any reason in connection
of your authorized use of my physical likeness as herein provided.
Refund
Policy:
Due to the portion of fees that are designated for course supplies,
full refunds can only be granted before and up to the start of
the first day of the course. If refund is requested after the
start of the first day of the course, five hundred dollars ($500.00)
will be refunded, however the remainder of fees will be non-refundable.
There are absolutely no refunds after the first day of the course. |
